2. Walk-ins are welcome. We are taking every precaution to ensure the safety and wellness of our staff and customers during the COVID-19 outbreak. We have implemented enhanced daily cleanliness and sanitation protocols, and are operating under social distancing guidelines. When visiting our location, we ask that you please put a minimum of 6 feet between you and anyone else present. We also ask that you bring a mask to wear while visiting, or ask us for one when you arrive. Our team members are required to wear a mask as well. The Public Health Institute at Denver Health offers multiple ways to order a birth certificate or death certificate including online, in person, or by mail. If you live near Denver and want to visit our office, we are centrally-located and offer quick and easy access.

Whether you are looking to order a new birth certificate, a birth certificate replacement, or a copy of a birth certificate or death certificate, we are here to help.

We issue the following records:

  • Certified birth certificates for the state of Colorado from 1905 - present.
  • Certified death certificates for Denver County from 1964 - present.

Who can order a birth or death certificate?

  • The person named on the birth record, with approved forms of identification.
  • Members of the immediate family, with required proof of relationship. To prove your relationship, we accept:
    • Birth certificates.
    • Marriage certificates.
    • Court-approved guardianship paperwork.
  • Legal representatives of those named above.

You can order certificates online, in person or by mail.

In Person

If you would like to order a birth or death certificate in person, please visit us at 120 W. 5th Ave., Denver, CO 80204. You will be asked to complete a form, and to present your current driver's license, state identification, passport or other form of accepted identification. (Español).


By Mail

To request a birth or death certificate by mail, please fill out the birth certificate request form, or death certificate request form, and mail the signed application, fee and photocopies of the appropriate proof of identity or relationship to our office at:

Mail requests will require one week processing time after we receive it.

Important Information
NOTICE On December 19, 2018, the Colorado Department of Public Health and Environment's (CDPHE) Board voted to allow more identifying options for birth certificates. There will now be four options: male, female, X and intersex. To make changes on a birth certificate, contact CDPHE.
Colorado Adoptees CDPHE began processing requests for original birth certificates for Colorado adult adoptees on January 4, 2016. These requests are only being handled by the Colorado Vital Records office. For more information and to obtain the request form, visit the CDPHE website, or call (303) 692-2200
Vital Records Requirements Receive birth or death certificates by in-person, or by mail. A valid driver's license is required. If you do not have a valid driver's license, valid identification will be required. For more information, please call (303) 602-3660 or fax (303) 602-3665.

Who is eligible to purchase a birth certificate?

You may purchase your own, or one for your:

  • Mother
  • Father
  • Children
  • Spouse
  • Siblings (birth certificate proving one common parent required)
  • Grandparents
  • Grandchildren

I was not born in Colorado. Can I get a birth certificate from your office?

No. You must contact the State Vital Records Office in the state in which you were born.
